For a child to bear the surname of his father, there are several options depending on the particular case of these is the legitimation process (parents married after s é baby's birth) the other option or n is the voluntary paternity acknowledgment. In the form of standing, used to add the biological father's name when he marries the mother after the birth of his son, a space appears to indicate a desire to change child's surname.
For the second option or n there is also a box on the form to indicate the desire to change the child's name in the form of Voluntary Acknowledgment of Paternity, when it is used to establish paternity.
However sometimes paternity is established by a court or administrative order issued by a court, you need to request in writing and signed by both parents, in order to change the surname of the child, within a deadline set by the court or administratively. This request must contain the name of the child and must be written in the new birth certificate. including administrative order or a certified copy of the ruling of the court which issues it.
